In addition to the services delivered by the clinical staff, the Del Corazon Care Team includes a Psych-social team of professionals.
A Medical Social Worker is available to help identify community resources which our hospice patient may be eligible to receive including financial assistance, delivery of meals, and if needed placement in a care facility. Assists the patient and family to draft Advanced Medical Directives, Living Wills, and issues relating to responsibilities of Power of Attorney. If requested, will assist in funeral planning.
Our Spiritual Care Coordinator / Chaplain offers support to patients and family members to identify, define, and better understand their spiritual needs in relationship to the pending passing. The chaplain provides assistance to whatever degree the patient wishes and can, when asked, serve as a spiritual companion. The hospice chaplain also may conduct funeral or memorial services.
The Bereavement Coordinator will provide the patient and/or family with pre-bereavement support to help prepare for the passing. Bereavement support to the patient’s loved ones is provided up to 13 months after the patient’s death.
The Volunteer Coordinator assigns and supervises Companion Volunteers to provide support and companionship to the patient and to family members.
